Denise Lisac Scholarship
Denise Lisac was the kind of teacher most aspire to be. She impacted students many times in many ways. For some, that impact may have been in the classroom; for others, it may have been on the field, in the hallways, or even outside of school. Denise helped shape many students and adults who came in contact with her and helped them become their best. In recognition of the many students and staff she touched, this scholarship is a way to carry on her legacy for years to come.

IMPORTANT SUBMITTAL INSTRUCTIONS
You must submit your scholarship while logged into gmail or google with your personal gmail/google account (not your student login) as follows:
1. At school, come to the Career Center or use the computers in the library to submit your application using your personal google/gmail account.
2. Whether at school or at home, press the Submit button.
3. If you are not logged into gmail or google, the usual google login will display. Login with your personal google/gmail account (not your student login).
4. Proceed by filling in the Scholarships Application Submission form and then upload your filled-in application and any required attachments. Once uploaded, you are returned to the Scholarships Application Submission form. Press the Submit button.
The scholarship
Two $1,000 scholarships will be made available to one male and one female David Douglas athlete who exemplifies the qualities possessed by Denise. These qualities include:
Giving
Responsibility
Integrity
Tenacity = GRIT

GPA REQUIREMENT – Must be eligible to meet Post-secondary requirements
Good citizenship
Overcame obstacles/barriers
Post-secondary bound
Special preference will be given for the following:
Four-year participant in athletics or activities
Honor student – 3.5 GPA or above
First in family to attend college or post-secondary program
A history of volunteering
Participant in a College Ready Program, such as College Possible, Upward Bound, etc.
Additional documentation required
Three Letters of recommendation; one from a coach/director and two from teachers.
A copy of high school and community college (if applicable) transcripts.
Written essay on the topic of the “Importance of Giving Back.” Essay should be word-processed and 1,000 words or less.
A short essay addressing an obstacle that the student has overcome at some point in their lifetime. The essay should be word-processed and 500 words or less.
